PROGRAM/LOCATION:       Re-Entry Rapid Re-housing/Sobrato House Youth Center

SUPERVISOR:   Program Coordinator      

STATUS:           Full-time, Non-exempt, Regular  

 

      

 

PROGRAM SUMMARY: 

The goal of HomeFirst’s Re-Entry Rapid Re-housing (RRH) program is to provide housing stability to homeless individuals who have been involved in the criminal justice system in the County of Santa Clara, CA. The agency’s Re-Entry RRH program will achieve this goal by providing participants with comprehensive supportive services through outreach and engagement, intensive, individualized case management, housing search, employment support, and temporary financial assistance.

 

DUTIES/RESPONSIBILITIES:

Direct Service (70%)

  • Maintain a case load as a case manager providing intensive case management and other

        direct service to clients

  • Work with program participants to develop and implement an individual case management

        And budgeting Plan, provide comprehensive support to help client achieve their set goals

  • Meet regularly with program participants at a frequency determined by the clients’ needs
  • Maintain thorough, accurate records of case management activities with every program

        participant, as required by grant contracts

  • Properly report any suspected cases of abuse
  • Conduct and complete all necessary documentation pertaining to HomeFirst and Re-Entry

RRH compliance and requirements 

  • Maintain accurate records of financial support given to clients and process financial

        assistance requested in a timely manner

  • Conduct outreach for individuals referred and those connected to the Re-Entry Center
  • Collaborate with team members to provide participants wrap-around support services
  • Establish and maintain productive working relationships with other departments
  • Provide appropriate referrals to external community services
  • Collaborate with community partners regarding support services for participants
  • Communicate effectively with Re-Entry staff and Probation/Parole officers
  • Engage in community-wide coordinated assessment efforts and documentation
  • Maintain professional relations and conduct with partners and external providers
  • Know and implement agency and program policies and procedures
  • Meet the program service delivery goals and outcomes
  • Utilize supervision appropriately, while maintaining open lines of communication and

        providing updates on participants and documentation

  • Work with individual landlords and other team members, as appropriate, to address client

        tenancy issues/problem(s)

  • Refer client to eviction defense legal services as appropriate
  • Provide case management sessions in client’s home and within the community as needed
  • Assist with other duties assigned

 

Documentation (20%)

  • Document, via HMIS, all direct services provided to guests within 3 business days and

        maintain all other case file paperwork and documentation

  • Conduct ongoing re-assessments of the clients’ needs and self-sufficiency, and document

        assessment in HMIS

  • Maintain up-to-date internal service tracking records
  • Participate in ongoing analysis of and program alignment with industry best practices
  • Proactively and consistently improve the quality and effectiveness of program services

 

General (10%)

  • Attend meetings, workshops and trainings as needed
  • Assist with office-related tasks as needed

 

QUALIFICATIONS:   

Education and Experience

  • Bachelor’s degree in Social Work or a related human services field required and a minimum of 1 year of relevant work experience (Bachelor’s degree may be substituted by a high school diploma or GED and an additional 2 years of relevant work experience)
  • MSW preferred

 

Skills, Abilities, and Knowledge

  • Strong written and oral communication skills
  • Exceptional organizational skills
  • Proficient with Microsoft Office suite
  • Ability to work with people of diverse socio-economic backgrounds
  • Experience in developing relationships with community partners and outside agencies
  • Ability to manage multiple projects concurrently and be flexible with priorities
  • Demonstrated ability to develop and maintain priorities and meet established deadlines
  • Ability to work as a member of a team
  • Ability to present oneself and the agency professionally to internal and external audiences
  • Strong understanding of Re-Entry issues
  • Preference for extensive field work over office-based work
  • Enjoy providing hands-on instruction in basic living and financial skills
  • Understanding of fair housing law and tenants’ rights and responsibilities
  • Understanding of and sensitivity to the needs of the homeless
  • Understanding of and strong adherence to social work ethics
