PROGRAM SUMMARY:  

The goal of HomeFirst’s supportive services is to provide access to resources and support for individuals who are at-risk of becoming homeless, or who are facing homelessness. The supportive services will empower individuals to better-manage their daily lives, while promoting self-sufficiency in order to obtain housing.  The program will offer comprehensive supportive and clinical services to assist individuals with life skills, and to link individuals to a network of community resources including, but not limited to, psychiatric services, medical services, employment resources, housing resources, and income resources.

The Clinician will work directly with the Bridge Housing Community (BHC) Program Manager and Associate Support Services Director to identify the clinical needs of and to provide services for BHC program participants. This may include, but is not limited to, individual therapy, group therapy, crisis support, drop-in hours, and skill building workshops. BBS Supervision available.

 

The BHC program, commonly referred to as the “Tiny Homes” project, will serve homeless individuals in newly developed communities in San Jose. Each community will consist of 40 sleeping units (aka Tiny Homes), shared restrooms, laundry, showers, and kitchen in a centralized building, along with a multi-purpose room and HomeFirst staff office space. 

HHHomeFirst will manage the property and offer on-site support services to the individuals residing at the BHC. Additionally, HomeFirst will engage the surrounding community to become involved in the BHC program. The target population for this program are individuals currently enrolled in Rapid Rehousing Programs (a county-wide program designed to quickly house homeless individuals and families with short term rental subsidies) with either HomeFirst or other service providers. Individuals participating in the BHC program are offered interim housing in one of the BHC locations, usually for less than 3 months, while they secure permanent housing with the support of their Rapid Rehousing (RRH) case manager. Services offered on-site will be designed to supplement services being provided by the individuals RRH case manager. 

 

DUTIES/RESPONSIBILITIES:

Direct Services (70%)

  • Maintain a case load by providing individual and group interventions, as well as drop-in support and crisis support
  • Provide group sessions onsite
  • Collaborate with the case managers to address items in the individual action plan, and provide support to help the individuals achieve their goals
  • Conduct ongoing re-assessments of the clients’ needs and levels of self sufficiency
  • Properly report any suspected cases of abuse
  • Provide coaching and training on topics of interest for program /agency staff

 

 

Documentation (20%)

  • Maintain thorough, accurate records of case management activities with every program participant in HMIS as required by grant contracts and internal compliance
  • Document all case management activity and progress notes in HMIS within 3 business days, and maintain all other case file paperwork and documentation
  • Complete and submit incident reports and behavior contracts, as needed
  • Conduct ongoing re-assessment of client needs and document in HMIS

 

General (10%)

  • Attend all meetings relevant to this position, including team meetings, agency trainings, and off-site trainings
  • Participate in opportunities for learning and skill maintenance/development, including internal and external training and workshops
  • Know and implement agency and program policies and procedures
  • Maintain professional relations and conduct
  • Utilize supervision appropriately, maintaining open lines of communication and providing updates on individuals
  • Participate in ongoing analysis of and program alignment with industry best practices
  • Proactively and consistently improve the quality and effectiveness of program services
  • Assist with other duties assigned

 

 

QUALIFICATIONS:

 

Education and Experience 

 

  • Master’s Degree in Social Work or Counseling, required
  • Registered with the Board of Behavioral Sciences as an Associate Marriage & Family Therapist (AMFT), or Associate Social Worker (ASW), required

 

Skills, Abilities, and Knowledge 

 

  • Strong understanding of PTSD, mental health symptoms, DSM 5 diagnoses, and chronic homelessness issues
  • Knowledgeable on Trauma Informed Care, CBT, and other evidence-based practices
  • Ability to quickly build trust and rapport in interpersonal relationships
  • Strong written and verbal communication skills
  • Independent problem-solving skills and the ability to prioritize multiple tasks
  • Demonstrated ability to develop and maintain priorities and meet established deadlines
  • Exceptional organizational skills
  • Strong sense of accountability
  • Ability to present oneself and the agency professionally to internal and external audience
  • Ability to work as a member of a team and can set and meet individual goals
  • Understanding of and sensitivity to the needs of the homeless population, or formerly homeless population
  • Experience in and interest in facilitation of groups
  • Comfortable with using computer software and databases
  • High level of flexibility and ability to prioritize appropriately 
  • Ability to work with people of diverse socio-economic backgrounds.
  • Enjoy providing hands-on instruction on life skills
  • Physical duties will include walking, sitting, ability to lift 25lbs 
  • Valid CA driver license and auto insurance
  • Reliable personal vehicle
