PROGRAM SUMMARY:  

HomeFirst Citywide Outreach and SOAR Project is designed to connect street and encampment homeless individuals to shelter, case management, mental health services, and housing. The target populations for this program are vulnerable, chronically homeless individuals who are living on the streets or in encampments throughout San Jose. Program staff will provide a consistent presence on the streets and other outdoor locations throughout the City of San Jose to build rapport and trust with unsheltered homeless residents. The Citywide Outreach and SOAR team includes Outreach Specialists, Case Managers, Drug and Alcohol Counselors, Clinicians and Clinical Supervision, Program Manager and Director.

 

The goal of HomeFirst’s supportive services is to provide access to resources and support for individuals who are at-risk of becoming homeless, or who are facing homelessness. The supportive services will empower individuals to better-manage their daily lives, while promoting self-sufficiency in order to obtain housing.  The program will offer comprehensive supportive and clinical services to assist individuals with life skills, and to link individuals to a network of community resources including, but not limited to, psychiatric services, medical services, employment resources, housing resources, and income resources.

Clinician will work directly with the Clinical Services Director to identify the clinical needs of and to provide services for Outreach SOAR participants at various encampments throughout the city of San Jose. This may include, but is not limited to, individual therapy, group therapy, crisis support, drop-in hours, and skill building workshops. BBS Supervision available.

 

 

DUTIES/RESPONSIBILITIES:

Direct Services (70%)

  • Maintain a case load by providing individual and group interventions, as well as drop-in support and crisis support at various encampments throughout the City of San Jose
  • Provide group sessions onsite at various encampments throughout the City of San Jose and HomeFirst locations
  • Collaborate with the SOAR Outreach case managers to address items in the individual Housing Action Plan, and provide support to help the individuals achieve their goals
  • Conduct ongoing re-assessments of the clients’ needs and levels of self sufficiency
  • Properly report any suspected cases of abuse
  • Provide coaching and training on topics of interest for program /agency staff

 

 

Documentation (20%)

  • Maintain thorough, accurate records of clinical activities with every program participant in HMIS as required by grant contracts and internal compliance
  • Document all activity and progress notes in HMIS within 3 business days, and maintain all other case file paperwork and documentation
  • Complete and submit incident reports and behavior contracts, as needed
  • Conduct ongoing re-assessment of client needs and document in HMIS

 

General (10%)

  • Attend all meetings relevant to this position, including team meetings, agency trainings, and off-site trainings
  • Participate in opportunities for learning and skill maintenance/development, including internal and external training and workshops
  • Know and implement agency and program policies and procedures
  • Maintain professional relations and conduct
  • Utilize supervision appropriately, maintaining open lines of communication and providing updates on individuals
  • Participate in ongoing analysis of and program alignment with industry best practices
  • Proactively and consistently improve the quality and effectiveness of program services
  • Assist with other duties assigned

 

 

QUALIFICATIONS:

 

Education and Experience

  • REQUIRED Master’s Degree in Social Work or Counseling
  • REQUIRED Registered with the Board of Behavioral Sciences as an Associate Marriage & Family Therapist (AMFT), or Associate Social Worker (ASW)

 

Skills, Abilities, and Knowledge 

 

  • Strong understanding of PTSD, mental health symptoms, DSM 5 diagnoses, and chronic homelessness issues
  • Knowledgeable on Trauma Informed Care, CBT, and other evidence-based practices
  • Ability to walk long distances on rough terrain (such as trails, creek sides and high traffic areas)
  • Preference for extensive field work over office-based work
  • Ability to quickly build trust and rapport in interpersonal relationships
  • Strong written and verbal communication skills
  • Independent problem-solving skills and the ability to prioritize multiple tasks
  • Demonstrated ability to develop and maintain priorities and meet established deadlines
  • Exceptional organizational skills and strong sense of accountability
  • Ability to present oneself and the agency professionally to internal and external audience
  • Ability to work as a member of a team and can set and meet individual goals
  • Understanding of and sensitivity to the needs of the homeless population, or formerly homeless population
  • Experience in and interest in facilitation of groups
  • Comfortable with using computer software and databases
  • High level of flexibility and ability to prioritize appropriately 
  • Ability to work with people of diverse socio-economic backgrounds.
  • Enjoy providing hands-on instruction on life skills
  • Physical duties will include walking, sitting, ability to lift 25lbs 
  • Valid CA driver license and auto insurance
  • Reliable personal vehicle
