PROGRAM SUMMARY:

The goal of HomeFirst’s North County Care Coordination Project (NC CCP) is to provide permanent housing stability and maintenance for chronic homeless individuals and families in the northern part of Santa Clara County, CA. The agency’s North County Care Coordination Project will achieve this goal by providing participants with comprehensive supportive services to help them maintain or attain permanent housing.  Intensive, individualized and therapeutic case management services coupled with linkage to a network of community resources (psychiatric, medical, and otherwise), assist clients to avert homelessness, maintain housing, and increase self-sufficiency.  The North County Care Coordination Project is Medi-Cal certified and will be providing therapeutic services to program participants starting January 2017. 

 

 

DUTIES/RESPONSIBILITIES:

Direct Services (70%)

   Maintain a case load as a case manager of persistently homeless individuals in North Santa Clara County by providing intensive case management and other direct service to clients

   Provide community-based case management services to program participants

   Work with landlords, including other team members, as appropriate, to address client tenancy issues/problems

   With the Program Coordinator, maintain accurate records of financial support given to clients and process financial assistance requests in a timely manner

   Collaborate with community partners regarding support services for clients

   Utilize supervision appropriately, maintaining open lines of communication and providing updates on intake activity

   Know and implement agency and program policies and procedures

   Properly report any suspected cases of abuse

   Maintain professional relations and conduct

   Maintain thorough, accurate records of case management activities with every program participant, as required by grant contracts

   Conduct client intake per HomeFirst and County requirements, including Vulnerability Index & Service Prioritization and Decision Making Tool (VI-SPDAT), Initial Mental Health Assessments, and other Medi-Cal documentation

   Work with program participants to develop and implement an individual case management plan, and provide support to help them achieve their goals

   Meet regularly with program participants at a frequency determined by the clients’ needs

   Provide appropriate referrals to other community services

   Assist clients with transportation needs to critical appointments

·   Conduct outreach in the field to locate clients, often at parks, along the creeks, and at encampments throughout Santa Clara County

·   Support client access and maintain Medi-Cal eligibility through re-certifications

·   Work in partnership with CCP team to develop Treatment Plans and assessments

·   Assist with other duties assigned

Documentation (20%)

   Document, via progress notes in HMIS, all case management activity within 3 business days and maintain all other case file paperwork and documentation

·    Propensity to follow Medi-Cal standards in conducting and documenting assessment, treatment plans and program notes once trained

   Participate in ongoing analysis of and program alignment with industry best practices

   Proactively and consistently improve the quality and effectiveness of program services

   Conduct ongoing re-assessments of the clients’ needs and self-sufficiency, and document assessment in HMIS

General (10%)

   Attend all meetings relevant to the position, including weekly Care Coordination Project meeting with County and internal team

   Assist with office-related tasks as needed

 

 

QUALIFICATIONS:

Education and Experience

   Bachelor’s degree plus one year minimum of human services experience, or minimum of three years of experience in human services

Skills, Abilities, and Knowledge

   Strong written and oral communication skills

   Experience in developing relationships with community partners and outside agencies

   Ability to manage multiple projects concurrently and be flexible with priorities

   Demonstrated ability to develop and maintain priorities and meet established deadlines

   Exceptional organizational skills

   Strong sense of accountability

   Ability to present oneself and the agency professionally to internal and external audiences

   Ability to work as a member of a team and can set and meet individual goals

   Ability to work with people of diverse socio-economic backgrounds

   Understanding of and sensitivity to the needs of the homeless

   Strong understanding of PTSD and chronic homelessness issues

   Preference to provide extensive field work and office-based work

   Enjoy providing hands-on instruction in basic living and financial skills

·    Able to work evenings/weekends as needed

Other

·   Clean driving record, valid CA Driver’s License and current auto insurance

·   Reliable personal vehicle

·   Field work, which may include walking upstairs

·   Able to lift up to 30 pounds
